Five Star Rating

New Community Extended Care Facility earned an overall five star rating from Medicare.gov, which is the highest possible rating given to a skilled nursing facility. The overall five star rating takes into account the facility’s score across three categories: health inspections, staffing and quality measures.

Medicare.gov releases ratings for nursing homes across the United States on a quarterly basis via Nursing Home Compare, an online database that’s accessible to the public and shows how many stars a nursing home has achieved (on a scale of one to five) in the three categories plus an overall score.

New Community Extended Care is proud of earning a five star rating and our staff continues to display a commitment to excellence in nursing care.

Long-Term Care

When our loved ones are no longer able to care for themselves and require medical attention around the clock, it can be difficult to know where to turn. At New Community Extended Care Facility, we strive to deliver excellent medical care in a clinical environment that addresses the physical, emotional and spiritual needs of residents in need of both long-term and short-term care. Short-Term Care

Our comprehensive rehabilitation program offers patients physical, occupational, and speech therapies. Our rehabilitation department consists of highly qualified therapists from Activa Rehabilitation Services with years of rehabilitation experience. Additionally, our state-of-the-art rehabilitation gym is equipped with the latest in rehabilitation equipment. Our team is dedicated to providing the highest quality physical, occupational and speech therapy services, while still maintaining a “compassionate touch” when it comes to customer service and patient care.

Sub-Acute Care

When your loved one has suffered an injury, illness or degenerative disease, sub-acute care can offer another option to long-term care. Residents staying at our dedicated 30-bed sub-acute unit receive more intensive care and treatment until their condition has stabilized. Your loved one will receive care from a team of medical professionals in order to achieve the goal of returning to the community or transitioning to a lower level of care.

Alzheimer’s & Dementia

New Community Extended Care offers enhanced services specifically geared towards residents with Alzheimer’s disease and dementia. As part of the comprehensive care provided at New Community Extended Care, our caregivers received specialized training on how to care for residents facing diminished memory and cognitive functioning. We also hold onsite monthly Alzheimer’s Support Group meetings  designed for caregivers to connect with each other and our staff.

Continuing Education

Staff at Extended Care are specially trained to work with individuals diagnosed with Alzheimer’s disease and dementia through ongoing workshops and seminars that provide continuing education credits for healthcare professionals. The workshops and seminars are free of cost and are led by qualified nursing home staff.
