New Community Extended Care has skilled therapists to administer physical therapy, occupational therapy, speech therapy and rehabilitation utilizing required equipment and compassionate care to ensure person-centered services.
New Community Extended Care’s rehabilitation program is unique because it offers compassionate care geared toward people who have faced health disparities with complicated comorbidities and gets them back to as near normal as possible. There is also follow-up after discharge by the facility’s Social Services Department to ensure continuous therapy if needed to maintain safety at home.
Getting people back to their pre-illness health within a short time through all of the above-mentioned services has always been our utmost goal at New Community Extended Care. With our follow-up post-discharge, most of our patients avoid re-hospitalization.
