As a 180-bed skilled nursing and subacute rehabilitation facility, we cater to both long-term residents and those requiring short-term, intensive care. Our 30-bed subacute unit is specially designed to facilitate a swift recovery. With a team of board-certified physicians and specialists in various fields, including dentistry, optometry, podiatry, psychiatry, psychology, surgery, hospice and respite care, X-rays and laboratory services, we ensure comprehensive on-site healthcare services for every individual under our roof.
New Community Extended Care provides skilled nursing, subacute rehabilitation, hospice care and respite care.
New Community Extended Care accepts Medicaid, Medicare and various HMO plans.
New Community Extended Care can assist with Medicaid applications, however, patients must provide the necessary documents, including five years of bank statements, Social Security award letters, proof of address, proof of identification, life insurance policies if applicable, pensions and annuities if applicable, legal U.S. citizenship, etc.
New Community Extended Care offers hospice care and palliative care for those in advanced stages of illness.
New Community Extended Care offers respite care to help families who will be out of town for a short time. The facility cares for the patient until the family members return.
