Nursing Services
New Community Extended Care is committed to providing the highest quality of nursing care 24 hours a day, seven days a week. We have comprised a team of experienced professionals who provide around-the-clock nursing care to all our residents, including those in advanced stages of illness. We are a 180-bed skilled nursing facility as well as a short-term subacute rehabilitation facility, with a dedicated 30-bed subacute unit. Our team consists of board certified attending physicians plus a multitude of consultant physicians specializing in dentistry, optometry podiatry, psychiatry, psychology and surgery. We also offer hospice and respite care, x-ray and laboratory services all on site.
Long-Term Care
When our loved ones are no longer able to care for themselves and require medical attention around the clock, it can be difficult to know where to turn. At New Community Extended Care Facility, we strive to deliver excellent medical care in a clinical environment that addresses the physical, emotional and spiritual needs of residents in need of both long-term and short-term care. Extended Care focuses on providing the best possible care through a team approach. Our multidisciplinary team consists of physicians, registered nurses, therapists, a psychiatrist and dietitian who all work together to provide customized care for your loved one.
Short-Term Care
Our comprehensive rehabilitation program offers patients physical, occupational, and speech therapies. Our rehabilitation department consists of highly qualified therapists from Activa Rehabilitation Services with years of rehabilitation experience. Additionally, our state-of-the-art rehabilitation gym is equipped with the latest in rehabilitation equipment. Our team is dedicated to providing the highest quality physical, occupational and speech therapy services, while still maintaining a “compassionate touch” when it comes to customer service and patient care.
Sub-Acute Care
When your loved one has suffered an injury, illness or degenerative disease, sub-acute care can offer another option to long-term care. Residents staying at our dedicated 30-bed sub-acute unit receive more intensive care and treatment until their condition has stabilized. Your loved one will receive care from a team of medical professionals in order to achieve the goal of returning to the community or transitioning to a lower level of care.
Alzheimer's & Dementia
New Community Extended Care offers enhanced services specifically geared towards residents with Alzheimer’s disease and dementia. As part of the comprehensive care provided at New Community Extended Care, our caregivers received specialized training on how to care for residents facing diminished memory and cognitive functioning. We also hold onsite Alzheimer’s Support Group meetings every three months designed for caregivers to connect with each other and our staff.
