New Community Extended Care Facility received a Five-Star Rating in September 2025 for Overall Quality and Quality Measures from the Centers for Medicare and Medicaid Services (CMS). Five stars is the highest rating that CMS gives. We at Extended Care are proud of our staff members for their hard work and dedication to quality care for all of our residents and patients.
